As a Care Coordinator, you will be the first voice a patient or their family hears when reaching out to Sailor Health. You will guide older adults and their caregivers through the intake process with warmth, clarity, and professionalism, ensuring they feel supported every step of the way.

This role is a unique blend of patient support, care coordination, and emotional guidance. You will be the steady hand helping each new patient navigate the journey into care, from matching with the right therapist to feeling confident and understood.

You will join a remote-first team that stays closely connected with a thoughtful, collaborative culture rooted in purpose. As part of a small, agile team, you will take initiative, shape key processes, and grow as the company grows.

Requirements

Empathetic and clear communicator who speaks with kindness and listens actively
Highly organized and detail-oriented, able to juggle multiple patients at different stages
Tech-comfortable with tools like Google Workspace, EHR platforms (e.g., Healthie), or CRMs like HubSpot
Mission-aligned and deeply interested in improving mental health care for older adults
Ability to make others feel at ease during difficult or emotional conversations
Experience in healthcare, customer service, or administrative support is welcome but not required

Responsibilities

Guide older adults and their caregivers through the intake process with warmth, clarity, and professionalism
Serve as the first emotional point of contact for individuals seeking mental health care
Match patients with the right therapist and ensure they feel supported every step of the way
Manage patient intake documentation and maintain accurate records in EHR platforms
Coordinate with the clinical team to ensure smooth transitions into care
Provide ongoing support and follow-up check-ins to patients and their families
Handle multiple patients at different stages of the process without letting things slip through the cracks
