Requirements

Bilingual fluency in Spanish and English
Experience outreaching high-need patients in Medicaid or Medicare settings
Knowledge of Serious Mental Illness and Substance Use Disorders
Familiarity with Enhanced Care Management practices
Ability to provide whole-person care across clinical, behavioral, and social domains
Passion for serving underserved populations
Empathy and creative problem-solving in care coordination
