Directly outreach and engage individuals living with Serious Mental Illness and Substance Use Disorders. Play a critical role in our whole-person, interdisciplinary care model. Think creatively and empathically to change primary care operations for those who need it most.

Requirements
- Bilingual fluency in Spanish and English
- Experience outreaching high-need patients in Medicaid or Medicare settings
- Knowledge of Serious Mental Illness and Substance Use Disorders
- Familiarity with Enhanced Care Management practices
- Ability to provide whole-person care across clinical, behavioral, and social domains
- Passion for serving underserved populations
- Empathy and creative problem-solving in care coordination
