The Referral Coordinator assists in researching and connecting with referral resources on behalf of participants accessing services.
The Referral Coordinator documents outreach and facilitates coordination with behavioral health treatment providers, facilities, and sponsored benefits. Key duties include researching community resources based on clinical recommendations and making referral arrangements for participants. This ensures participants receive appropriate support and care.
This is a remote position allowing work from anywhere in the US. The role involves attending staff meetings and participating in a required monthly Saturday rotation. Emphasis is placed on documenting quality assurance issues and participant satisfaction with providers.
Daily activities focus on troubleshooting access issues and connecting clients to employer or community benefits. All activity completed during the day must be documented prior to leaving in the customized care management platform. This supports exceptional client satisfaction and service delivery.
Requirements
- Associate degree in Business, Wellness, Healthcare, or related field required
- Experience utilizing a case management platform
- Comfortable making many outbound phone calls and inquiries each day
- Experience in Microsoft Office, Word, Excel, PowerPoint and Outlook
- Familiarity with behavioral health treatment providers and community resources
Responsibilities
- Facilitate scheduling of participants and/or live transfers to the team
- Research and contact community resources/treatment providers based on clinical recommendations
- Identify materials which would provide additional support and/or education to participants
- Make referral arrangements for all participants
- Document all referrals researched and provided to participants
- Document quality assurance issues/participant satisfaction with treatment providers and facilities
- Review and update referral information in case management platform
- Identify participant access issues, troubleshoot, and assist to ensure care is obtained
